How PRP Works:

  1. Blood Draw: A small amount of blood is drawn from your arm, similar to a routine lab test. (At SD PRP Clinic, we draw between 30 to 120 mL of blood depending on the treatment area, with 60 mL being standard for most protocols).
  2. Centrifugation: Your blood is placed into a specialized processing system and centrifuge, which spins rapidly to separate its biological components. This isolates and concentrates the healing platelets. (At SD PRP Clinic, we exclusively utilize the gold-standard, FDA-cleared EmCyte SP kits).
  3. Targeted Injection: The superphysiologic platelet-rich plasma (PRP) is injected under advanced real-time ultrasound guidance directly into the site of injury or pain, ensuring precise delivery to damaged tendons, ligaments, or joint spaces.

Potential Benefits:

  1. Targeted Pain Reduction: PRP has been clinically shown to reduce chronic pain and inflammation in stubborn tendon injuries and arthritic joints, offering an appealing alternative for patients struggling with persistent discomfort.
  2. Accelerated Tissue Healing: The concentrated growth factors found in platelets stimulate cellular proliferation, collagen synthesis, and neovascularization to help repair damaged soft tissues throughout the body.
  3. Restored Mobility & Function: Many patients experience significant improvements in joint function, grip strength, and range of motion following therapy, leading to a better quality of life and a reduced reliance on long-term medications or surgery.

Most Common Conditions We Treat with PRP-A2M

  • Osteoarthritis of Knee, Shoulder, Hip, etc:  Particularly in the early or mid-stages, PRP with A2M may help reduce pain and slow down the progression of cartilage degeneration. PRP is not advised in servers osteoarthritis warranted a joint replacement surgery (can be tried if surgery is not possible).
  • Tendinitis/Tendinosis : PRP is used to treat tendon injuries. For example in the knee when there is a patellar tendinitis, or in elbow for tennis elbow
  • Ligament Injuries/Partial tendon tear (ie Rotator Cuff) : Studies suggest PRP can help heal shoulder and knee ligament injuries and heal partial tendon Teras (the evidence is still mixed).

Other Musculoskeletal Conditions We Treat with Ultrasound Guided PRP/ A2M

  • Tennis Elbow (Lateral Epicondylitis): Effective treatment for chronic tendon degeneration at the outer elbow.
  • Golfer’s Elbow (Medial Epicondylitis): Targeted healing for forearm flexor tendonosis.
  • Achilles Tendonitis & Tendinopathy: Promotes repair for chronic micro-tears and inflammatory degeneration in the Achilles tendon.
  • Plantar Fasciitis: Relief for stubborn, chronic heel pain unresponsive to conventional stretching or orthotics.
  • Ligament Sprains & Partial Tears: Supports healing for stability structures such as the Ulnar Collateral Ligament (UCL) in the elbow or minor joint instability.
  • Muscle Strains & Sports Injuries: Accelerated recovery for major muscle-belly tears, hamstring strains, and calf pulls.
  • AC Joint Pain & Arthritis: Targeted regenerative care for acromioclavicular joint degeneration.
  • Carpal Tunnel Syndrome & Wrist Pain: Supportive soft-tissue management for nerve compression and wrist discomfort.
  • Temporomandibular Joint (TMJ) Disorders: Precision Intra-articular PRP injections are used to repair damaged joint discs, reduce inflammation, and alleviate pain in patients suffering from degenerative TMJ conditions.
  • Masseter Muscle Pain / Myofascial Pain Syndrome: PRP is injected into trigger points within the masseter muscle to resolve chronic spasms, tension, jaw clenching pain (bruxism-related muscle fatigue), and myofascial pain.

Medical Conditions We Treat at SD PRP Clinic with PRP

  • Anal Fissures (Acute and Chronic): Intralesional PRP injections are increasingly used as a regenerative, minimally invasive alternative or adjunct to surgery. PRP promotes rapid epithelialization, reduces mucosal ischemia, and significantly decreases pain and bleeding associated with non-healing anal ulcers.
  • Pilonidal Sinuses / Fistulas: Sometimes utilized in specialized proctology to reinforce wound healing post-surgery and reduce recurrence rates.
  • Chronic Non-Healing Wounds: Applied to diabetic foot ulcers, venous leg ulcers, and pressure sores to jumpstart stalled cellular healing pathways.
  • Surgical Augmentation: Used during plastic or reconstructive surgery to improve flap survival and accelerate overall incision healing.
  • Peripheral Neuropathy: Nerve inflammation and localized chronic nerve pain.
  • Scar Remodeling & Acne Scars: Collagen stimulation to reduce depressed or hypertrophic scars.
  • Vulvovaginal Atrophy (VVA) /Vaginal Dryness/ Genitourinary Syndrome of Menopause (GSM):Thinning, drying, and inflammation of the vaginal walls typically caused by declining estrogen levels during or after menopause. PRP Injections into the vaginal walls and mucosa aim to stimulate collagen production, improve microvascular blood flow, and restore natural lubrication and tissue thickness. Clinical studies suggest it can reduce painful intercourse (dyspareunia) and improve overall vaginal health scores.
  • Plaque Psoriasis & Nail Psoriasis: Investigated for localized injection into resistant psoriatic plaques or nail matrices to modulate inflammation and restore nail plate integrity.

Note: While clinical data, case series, and randomized controlled trials show very promising results for many of these indications (especially orthopedics, chronic wounds, and anal fissures), efficacy can vary depending on individual patient health and the specific preparation method of the PRP.

Double-Spin PRP + A2M (The Complete Joint Protocol)

For majority of musculoskeletal conditions and osteoarthritis, SD PRP Clinic introduces Alpha-2-Macroglobulin (A2M) into the healing matrix of double spin PRP.

The Analogy: While PRP is the "construction crew" that rebuilds tissue, A2M is the "security guard" that protects the joint.

The Science: In a damaged or arthritic joint, destructive enzymes (catabolic proteases) constantly chew away at cartilage. A2M is a massive, naturally occurring plasma protein captured during our specialized filtration processing. It acts as a molecular trap, capturing and neutralizing these destructive enzymes.

The Process: This protocol involves a similar but more sophisticated process than double-spin PRP. We utilize a specialized concentrator filter to eliminate excess water, unwanted low-molecular-weight proteins, and residual red blood cells, isolating a highly concentrated dose of pure A2M. By combining Double-Spin PRP with A2M, we jumpstart tissue repair while actively shutting down the chemical processes causing joint degeneration.

Is It Effective?

The effectiveness of PRP can vary depending on many factors. Studies show positive results in reducing pain and improving function. The evidence is still growing, and outcomes can be inconsistent depending on several contributing factors (i.e post procedure resting/activity, the severity of the problem, the patient's overall health, and the technique used).

Frequently Asked Questions

How many treatments will I need?

While some patients experience significant relief after a single session, complex or long-standing chronic tendonopathies and arthritic conditions may require a series of 2 to 3 injections spaced several weeks apart.

Is PRP covered by health insurance?

No. Currently, regenerative medicine procedures like PRP and A2M are considered elective and are not covered by commercial insurance plans or Medicare. We provide transparent pricing during your initial consultation. You may use your HSA for these therapies.

What is the recovery timeline?

Because PRP relies on your body’s natural inflammatory and healing cascade, initial post-procedure soreness is normal for a few days. True structural tissue remodeling and functional improvements typically develop gradually over 4 to 12 weeks.
